Community Health Officer (CHO) Application Form 2024

The UP government invites nurses to CCHN training courses for the 2024-25 session. This course is under the initiative of the Ayushman Bharat Scheme. This course is started by the UP government to build up the Health and Wellness Centres(HWC’s). The NHM, UP application forms, will start from 15th August 2024 at 11:00 A.M and the last date to apply is 20th February 2024 till midnight. All the aspiring GNM candidates have to apply before the given time period.

The Uttar Pradesh government will conduct a written examination for the post of Community Health Officer after completing this 6 months Certificate in Community Health for Nurses (CCHN), candidates who clear the examination will be posted to Sub Health centers in the state as a Community Health Officer. This position is on a contract basis there is no official notification yet about the permanent post. It is expected that UP health authorities will offer permanent CHN jobs to the recruited candidates later.

UP NHM invites candidates with General Nursing and Midwifery (GNM), B.sc Nursing graduates, or Post B.sc Nursing course from any government recognized university to apply for the Community Health Officer (CHO) posts.

• The candidate must have a valid registered certificate as a registered nurse or Midwifery from UP Nurses and Midwives council.
• This certificate must be provided at the time of online registration.
